Crash Files offers a rare, inside look into the world’s foremost crash investigation agency: the National Transportation Safety Board. Each hour takes an in-depth look at a single tragic accident. We follow the fascinating process of how an NTSB “Go Team” is able to pieced together from a handful of clues a complete picture of what happened – and what can be done to keep it from ever happening again.

1. Korean Air 801 August 6, 1997—Korean Air 801 smashes into a hillside three miles Southwest of Guam’s Agana Airport as it approaches to land. Almost 2,500 rescuers negotiate treacherous terrain and bad weather conditions to reach the flaming debris, which lay in a ravine beneath the bluff. Of the 254 passengers on board, 228 are found dead, nut miraculously, 26 survive.

2.Silver Springs February 16, 1996—A Maryland Rail Commuter train collides with an Amtrak train killing 11. The impact derails both trains and ignites ruptured fuel tanks. Seventeen months later, investigators conclude that the accident was caused by a poorly designed signal system and subsequent failure by the commuter train to stop at a red light.

3. Valuejet 592 May 11, 1996—Valuejet 592, enroute from Miami to Atlanta, crashes into the Florida Everglades 10 minutes after takeoff, killing all 110 people on board. As a result, for the first time in U.S aviation history, manslaughter charges are brought against airline maintenance workers.

4. JFK, Jr. July 16, 1999—A single-engine Piper Saratoga aircraft disappears off the radar track and plunges into the Atlantic Ocean. On board are John F. Kennedy Jr., his wife Caroline Bessette Kennedy and her sister Lauren Bessette. The fatal crash was due to a pilot error.

5.Payne Stewart October 25, 1999—Golf Champion Payne Stewart, two of his agents and a friend are on their way to Dallas, Texas for a meeting when the Learjet that they are in suddenly begins flying at dangerously high altitudes, veering off its intended flight course. Military Fighter Jets, F-16’s,attempt to intercept, but are unsuccessful. Four hours later, after running out of fuel, the plane plummets into a marshy field, killing all on board.

6. The Concorde July 25, 2000-An Air France Concord, the world’s fastest commercial airliner, rolls down the tarmac to reach its take of f speed of 250 mph when the control radios, “Concord zero 4590, you have flames. You have flames behind you.” All passengers, flight crew and four people on the ground are killed.

7. Duffy Street Tragic accidents are mercifully rare- and rarer still are tragedies that strike twice. In May 1989, a runaway freight train, hurdling down a mountain pass at over 100 mph, derailed on Duffy Street, a quiet San Bernardino neighborhood, crushing seven homes and killing four people.

8. Sunset Limited The Sunset Limited Amtrak’s only transcontinental passenger train, running from Los Angeles to Miami. Late on a foggy September night in 1993, it pulled out of Mobile, Alabama headed east. Minutes later, as it reached its cruising speed of 70mph, it came to a bridge and without warning, seven cars- including two full of passengers-flew into the muddy bayou.

9. USAIR 427 September 8, 1994, a USAir Boeing 737 jet was approaching Pittsburgh, after a brief and uneventful flight from Chicago. Without warning, something caused it to bank sharply to the left. As the stunned pilots struggled to gain control, the jet corkscrewed 6000 feet downward, smashing into a wooded hillside at 250 mph, demolishing the plane and instantly killing all 132 people on board.
